Deregistering with SHAPE Medical Centre and EJSU Host Nation GP's

Please complete the deregistration form and return to the Medical Centre at SHAPE. Forms can be obtained from the Unit Administration office.

Deregistering from Sennelager Medical Centre

All patients in British Army (Germany) are advised to ring Sennelager Dental Centre on 05254 982 2482 or MIL 79 2482 and request to be cleared, ensuring they have their personal email address at hand. Once cleared, by telephone, with the Dental Centre, an email will be sent to the patient confirming clearance has been completed. This email confirmation is to be printed and attached the British Army (Germany) Clearance Certificate, in lieu of a signature from the Dental Centre. This should be presented to the Medical Centre upon de-registration.

Serving personnel

Visit your local medical centre to complete a deregistration form. Once complete, we will sign your Clearance Chit and at the appropriate time push your medical records to your next military practice. Your family member records will be held in the NHS central holding facility until they register in the UK when they will then be pulled to the new location.

Call your local medical centre to complete a de-registration form over the phone. We will e-mail you confirmation of clearance. This can be attached to your clearance chit instead of a signature.

Download the de-registration form here, complete it and send to your local Medical Centre. Ensure you include a current email address, so that a confirmation email can be sent to you, you can then attach our confirmation to your Clearance Chit in lieu of a signature.

Dependants and entitled civilians

Visit your local Medical Centre and complete a de-registration form. Your medical records will be sent to the central repository in the UK at the appropriate time. Once you re-register in the UK with a GP surgery your notes will be pulled to your new practice.

Call your local medical centre reception, and the staff will complete the de-registration form with you over the phone.
